Case report of nonsecretory multiple myeloma with acute onset of renal failure and nodular glomerulosclerosis.

37 year-old man, undergoing chronic hemodialysis therapy due to acute renal failure, showed some manifestations of myeloma. He had no abnormalities in immunoelectrophoresis and glucose tolerance test. He died of circulatory failure and post-mortem examination showed diffuse infiltration of myeloma cells. Diagnosis of non-secretory multiple myeloma was made.<BR> In the renal specimen from autopsy, half of glomeruli were completely hyalinized, and diffuse mesangial widening, quite similar to diabetic nodular glomerulosclerosis, was present in more than half of the remaining glomeruli. Electron microscopy confirmed the presence of massive, nodular, mesangial expansion consisting of finely reticular substances. Wrinkling basement membrane surrounded the nodules and electron-dense deposits were present on subendothelial side of the glomerular basement membrane. Electron-lucent materials were seen between the nodules and the glomerular basement membrane. Amyloid was not observed.
