A 14-year-old girl presented with complaints of dyspnea and dysphagia due to a huge antrochoanal polyp. Clinical examinations including X-ray and CT revealed a huge tumor located in the space of the epipharynx and mesopharynx. A rhinoscopic examination revealed that the tumor with a fine stem had originated from the middle nasal meatus. A Caldwel-Lucoperation was performed to remove the tumor along with the mucosa of the maxillary and ethmoid sinus. The tumor was diagnosed as an inflammatory polyp by histopathological examination. The weight of the polyp was 40 grams, making it the largest maxillochoanal polyp among cases reported in Japan.
