Two Cases of Acute Megakaryoblastic Leukemia in Childhood with Tumor Formation of the Orbital Region at a Relapse

Two cases of acute megakaryoblastic leukemia in childhood with tumor formation of the orbital region at a relapse were reported. A definite diagnosis of Case 1, a 16-month-old boy, was made on the electron microscopy investigation wherein PPO positivity was detected in 43% of bone marrow blast cells ; that of Case 2, a 7-month-old boy, was made on PPO positivity in 86% of bone marrow blast cells. Both cases were not accompanied with any underlying disease, such as Down's syndrome. Case 1 was treated with ACMVP, Case 2 with BHAC-DMP, and complete remission in both cases was obtained. However, 1 and a half year later in Case 1 and 3 months later in Case 2, r-exophthalmos appeared and the r-orbital tumor was found by CT in each case. Both cases were successfully treated with irradiation in addition to chemotherapy and tumors were reduced. Case 1 died of meningeal leukemia after surviving for 2 and a quarter years, whereas Case 2 died of destruction of the temporal bone after surviving for 9 months. Both cases had the orbital tumor and similar clinical course. These cases are very rare for acute megakaryoblastic leukemia.
