It is very important to avoid complications of the mandibular branch of the facial nerve in submandibular operations because the depressors of the corner of the mouth consists of the depressor labii inferior and the depressor anguli oris which are both supplied by the mandibular branch. In genaral, we have two approaches to preserve the nerve, the method to find the nerve and not to identify the nerve. The identified method has some problems when there is much fatty tissue under the platysma and there are several mandibular branches. The non-identified method is the approach to lift the skin flap with the platysma and the nerve along the capsule of submandibular gland to the edge of mandible. Though the non-identified method is good and safe approach to preserve the mandibular branch, it has no indication of being malignant lesion in submandibular region because of remaining some tissue on superficial layer. We investigated in this study the methods to preserve the mandibular branch of the facial nerve in various operations for submandibular lesion.
