Dermatofibrosarcoma protuberans (DFSP) is an uncommon, slow growing malignant tumor of the skin. DFSP often arises in the skin of the trunk or proximal limbs, but it is rare in the head and neck. We report two cases of DFSP in the head and neck. Case 1: A 61-year-old female had a slow growing mass for 20 years in the center of the anterior neck. A 40×32 mm tumor was diagnosed as DFSP by incisional cutaneous biopsy. She was treated by wide excision of the tumor with surgical margins of 3 cm and by reconstruction of the skin defect with a delto-pectral skin flap. Case 2: A 68-year-old male had a slow growing mass for 2 years on the cheek. A 15×15 mm tumor was diagnosed as DFSP by incisional cutaneous biopsy. He was treated by wide excision of the tumor with surgical margins of 1.5 cm in the upper lip and 3 cm in the other portion. The large skin defect was reconstructed with a cervico-facial skin flap. We successfully treated these two cases of DFSP without recurrence of tumor for 36 month and 14 months, respectively. Regional lymph node metastasis and distant metastasis of DFSP is very rare, but a high rate of local recurrence has been reported. It is important to excise widely at the first treatment.
