A Case of Rhinogenic Intracranial Complications

We report the case of a 23-year-old male with epidural and subdural abscesses secondary to acute frontal sinusitis. The patient complained a headache, vomiting, and anorexia. Intracranial extension may have developed via a direct route through a bone defect in the posterior wall of the frontal sinus. The subdural abscesses in the left frontal and right parietal regions were successfully treated by trephination and neurosurgical drainage. The epidural abscess in the right frontal regions was successfully treated by endoscopic sinus surgery. The pus in the epidural space was evacuated through the bone defect into the middle nasal meatus. Close monitoring by computed tomography onto intracranial and nasal regions is useful for deciding the timing of surgery and the approach. Endoscopic sinus surgery in combination with trephination and neurosurgical drainage is minimal and effective therapy for rhinogenic intracranial complications.
