A Case of Orbitofrontal Cholesterol Granuloma

The most frequent site of cholesterol granulomas in the head and neck region is the middle ear and mastoid air cells, and the orbitofrontal area is an uncommon site. We report a case of orbitofrontal cholesterol granuloma.A 40 year old man with no history of facial trauma or sinonasal surgery complained of proptosis of the right eye and diplopia for the past six months. A computed tomography scan revealed a large mass lesion in the superotemporal guadrant of the right orbit had caused bony destruction of the roof of tha orbit. Magnetic resonance images (MRI) demonstrated it to be a high-signal on T1-weighed images, and to be a high-signal mass containing an iso-signal area on T2-weighted. It was not enhanced by contrast medium. The imaging examinations showed no evidence of pathology, including evidence of inflammation and tumors, in any of the other paranasal sinuses. The mass lesion was totally resected through a bicoronal incision under general anesthesia. After removal, the cavity was obliterated with abdominal fat tissue, and the bony defect in the anterior wall of the sinus was reconstructed with a titanium mesh. The resected mass was diagnosed as a cholesterol granuloma based on the histopathological findings, incliding cholesterol clefts, multinucleated foreign body giant cells, and hemosiderins. Postoperatively, the orbital symptoms recovered. No recurrences or cosmetic deformities have developed during the 13 months since surgery.
