Prevalence Rate of Nasal Disease among MRSA Nasal Carriers

From 1988, we have surveyed nasal colonization with methicillin-resistant Staphylococcus Aureus (MRSA) in 734 medical staff (356 doctors, 378 nurses) at Yamanashi University Hospital. In 2003, MRSA colonization was detected in 35 medical staff: 12 doctors (3.4%) and 23 nurses (6.1%). In 31 of the 35 medical staff in whom MRSA was detected, we checked the prevalence rate of nasal allergy and chronic sinusitis. Nasal disease, such as nasal allergy or chronic sinusitis, has been thought to be one of the risk factors for MRSA colonization of the nasal vestibules because if a medical staff member has a nasal disease, he/she is likely to touch his/her nose frequently. However, there has been no evidence regarding this hypothesis.Nasal allergy was diagnosed from nasal symptoms indicative of nasal allergy and by intradermal skin tests using 4 allergens (HD, Japanese cedar, orchard grass, and ragweed). Chronic sinusitis was diagnosed from nasal symptoms indicative of chronic sinusitis and by nose X-rays.Nineteen medical staff (61.3%) had symptoms indicative of nasal allergy and 21 medical staff (67.7%) had a positive reaction to one or more allergens. These ratios were lower than those of medical students in the 5th year at Yamanashi University.None had symptoms indicative of chronic sinusitis, and only three had mild lesions in their maxillary sinus on the X-ray findings.From these data, the authors conclude that the hypothesis concerning nasal disease as one of the risk factors of MRSA colonization of the nasal vestibules has not been proven.
