Clinical Study of Frontal Sinus Surgery

The management of surgical treatment for frontal cyst or sinusitis has given inconsistent results due to anatomical variations. We studied 23 patients aged 11-88 years (mean: 54 years) who undergoing frontal sinus surgery for frontal cyst or frontal sinusitis at Niigata University Medical and Dental Hospital between January 1998 and December 2003. The most common symptoms in frontal cyst were double vision, ocular deviation, swelling, and exophthalmos, and all patients had pain in frontal sinusitis. Endonasal frontal sinusotomy was conducted in 13 (8 frontal cysts and 5 frontal sinusitis), and 10 with frontal cysts underwent combined internal and external approaches. Of the 14 who underwent use of the Silastic drainage tube, 6 straight tubes (6mm) and 8 T-shaped (4-6mm) tubes were used between the frontal sinus and nasal cavity. The straight tube was needed to stabilize sutures, whereas the T-shaped tube was not, and then it was easy to insert into or remove from the frontal sinus. The T-shaped tube had fewer complications with granulation, and it was possible to use for the long-term. T-shaped Silastic tubes should thus be chosen to prevent obstructing the front nasal outflow tract.
