Anatomical changes of the sinus after operation for chronic sinusitis were investigated. During the past 6 years, cases of re-operation for chronic sinusitis was 342 nasal sides of 201 patients. At the time of the initial surgery the most commonly applied technique had been endoscopic sinus surgery (ESS), and ESS was once again by far the most common technique used for the re-operation. Morphological changes were most often observed in the turbinate, i. e., in 75% of the total cases. It was surmised that, when preparing to re-operate on the paranasal cavity, surgeons must recognize that morphological changes in the turbinate are usually already present. The next most-common morphological changes were residual pneumatization or adhesion of the ethmoidal sinus, adhesion of the middle meatus, and dislocation of the lamina papyracea. The frontal sinus and the sphenoidal sinus were present in all cases, and these become accurate landmarks. There were 27 patients who had undergone polysurgery in the past and it is also necessary to take into account the psychological state of the patients.
