REGULATION OF EOSINOPHILIC APOPTOSIS IN A NASAL POLYP WITH EOSINOPHILIC INFILTRATION

Although the main treatment of chronic sinusitis is a macrolide therapy and operation therapy, the cases that show resistance to these therapy exist. It is known that there is many case that admit eosinophilic infiltration in a nasal polyp especially, and steroids administration are effective in those cases. Recently eosinophilic infiltration is related to the decrease of apoptosis of eosinophil. We studied apoptosis of the eosinophil in a nasal polyp. Local injection of betamethasone in the nasal polyp (2mg, two days before the operation), were given to a patient with severe chronic sinusitis, allergic rhinitis, and a nasal polyp with eosinophilic infiltration. We compared apoptosis and IL-5 in the eosinophils of the nasal polyp measured before injection and during operation. Identification of apoptosis was by the Tunel method, and that of IL-5, was done immunohistochemically. The tendency that the IL-5 positivity cells in a nasal polyp decrease after steroid administering was seen. Apoptosis increased significantly after the steroid was administered. In the majority of cases with increased apoptosis after steroid administering, the ratio of IL-5-positive eosinophil after administration was 50% or less. Therefore, a reverse correlation between the ratio of IL-5-positive eosinophils and that of eosinophilic apoptosis was admitted. It was suggested that IL-5 in eosinophils decreases apoptosis of eosinophils and eosinophilic infiltration in a nasal polyp increases.
