Surgical treatment of ruptured cerebral aneurysm for the aged patients

Long-term outcome of 39 patients more than 65 years of age who underwent direct operation of intracranial aneurysm was evaluated. Follow-up periods were 2 years and 10 months in average.<BR>The results were as follows : <BR>1) Eighteen patients (46%) lived in healthy lives without any neurological deficits and eight patients (21%) were self-cared with slight neurological deficits. However, four patients (10%) were severely disabled and were bed-ridden the whole day and nine patients (23%) died. The results indicated that twenty-six patiens (68%) took preferable outcomes.<BR>2) A rate of preferable outcomes following surgery was 90%, 45% and 0% in grade 1 and 2, grade 3 and grade 4, respectively.<BR>3) The causes of death were vasopasm in 3 patients, and general complications (pneumonia etc) in 6 patients.<BR>4) The authors concluded that the patients of grade 1 and 2 should be treated surgically as soon as possible, and also that early operation was indicated for the patients of grade 3 who lived in healthy life without any particular disease before occurrence of subarachnoid hemorrhage.
