We report a case of motor aprosodia who suffered from a brain infarction in the territory of the right middle cerebral artery. A 50-year-old right-handed woman suddenly noticed right hemiparesis, and it was found that she spoke like a Chinese. She was alert. Her speech was fluent but prosody was disturbed. The pitch of her speech was especially impaired. Postpositional particles of Japanese (jyoshi) were sometime omitted. She frequently employed a postpositional particle "ne" and spoke with a high-pitched sound at the end of words and sentences. Spontaneous prosody and prosodic repetition were markedly disturbed. However, auditory comprehension and prosodic comprehension were normal. Repetition, naming and reading were normal except for prosody. During reading, her pause was normal and no particles were omitted. Writing was normal. Her aprosodia was diagnosed as motor aprosodia according to the classification of prosody given by Ross. Brain CT and MRI revealed a brain infarction in the territory of the right middle cerebral artery. The lesion was in the posterior frontal, anterior parietal and upper temporal lobes including the basal ganglia. It is considered that the motor aprosodia had occurred due to the lesion in the posterior frontal lobe.
