Sequential changes of CT findings and outcome in patients with aneurysmal subarachnoid hemorrhage.

The clearance rate of cisternal blood was studied in 79 patients with Fisher's group 3 subarachnoid hemorrhage due to ruptured aneurysms of the anterior circulation. Each cistern was scored according to the amount of blood evident on computed tomography (CT), and the clearance rate was calculated from the sum score of each of the first two CT scans which were obtained within 2 weeks of onset. No significant correlation was observed between the clearance rate and symptomatic vasospasm, cerebral infarction, or normal pressure hydrocephalus (NPH), but there was a tendency for the clearance rate to be higher in patients without symptomatic vasospasm, infarction or NPH. The clearance rate was significantly higher in the patients with a better outcome at discharge. The clearance rate of cisternal blood on CT can thus be of prognostic value in patients with aneurysmal subarachnoid hemorrhage.
