A case of Moyamoya disease complicated with Basedow disease.

We reported a 35-year-old female of Moyamoya disease complicated with Basedow disease.<BR>She was admitted complaining of difficulty of speech. Neurological examination revealed a slight impairment of consciousness (JCS 1), transcortical motor aphasia, and minimal right hemiparesis. Endocrine function tests showed hyperthyroidism. Her clinical signs gradually improved after initiation of antithyroid therapy. Cerebral infarction and atrophy were obserbed on X-ray CT and MRI. The angiographic findings showed the features of Moyamoya disease.<BR>Some investigators have mentioned that the cerebral blood flow (CBF) in Moyamoya disease might be influenced by the superior cervical ganglion (SCG), and that CBF might decrease due to the stimulation of sympathetic ganglion. In this case, it is supposed that decrease in CBF and subsequent cerebral infarction resulted from the stimulation of sympathetic nerves, especially SCG, because of hyperthyroidism.<BR>It is concluded that appropriate treatment for thyroid function is important for prognosis of Moyamoya disease complicated with Basedow disease.<BR>The etiology of Moyamoya disease is said to be related to immunological reaction and sympathetic hypertonicity. Basedow disease is one of immunological illness, and stimulates sympathetic nerves. We believe that Basedow disease could be assosiated with causal genesis of Moyamoya disease.
