A clinical study on ischemic cerebrovascular disorders in polycythemia vera and essential thrombocythemia.

We conduced a clinical study on 5 cases of polycythemia vera and 2 cases of essential thrombocytosis accompanied by ischemic cerebrovascular disorders. The main signs included headache in 4 cases, splenomegaly in 6 cases, myocardial infarction in 2 cases, and arterial thrombosis in the extremities in 1 case. One patient with polycythemia vera died of a mixed-type cerebral hemorrhage after ingestion of aspirin, and in another case, cerebral ischemic attacks were observed a total of 5 times. CT findings in polycythemia vera included cortical branches, penetrating branch infarcts, but in essential thrombocytosis penetrating branch infarcts were not observed. MRI findings were similar to CT findings, though in 1 case of essential thrombocytosis an old cerebral hemorrhagic focus was detected. MRI is thought to be useful at the time of determining treatment. In treating, it is necessary to prevent cerebral hemorrhages by monitoring platelet aggregation capacity and hemorrhaging time during antiplatelet treatment. Also, it is possible to conduct thiamphenicol treatment on an outpatient basis, and this is thought to be one useful treatment method in the clinical management along with anti-platelet therapy in some cases of polycythemia vera and essential thrombocytosis accompanied by ischemic cerebrovascular diaorders.
