Changes in Serum Cytokines during Blood Autotransfusion of Patients with Abdominal Injuries.

We studied changes in blood cytokine levels after wash collection in salvaging autologous blood transfusion (SAT). Subjects were 15 patients with intraperitoneal hemorrhage divided into Group A (n=7), in which SAT was done and Group B (controls) (n=8) in which it was not. Peripheral blood was collected before and after surgery to measure interleukin (IL)-6 and 8, tumor necrosis factor (TNF) s-α, and IL-1 antagonistic substance (IL-1ra) by enzyme-linked immunosorbent assay (ELISA). In Group A, maximum blood collected during SAT was 4, 000ml; IL-6 was 141.6±38.6pg/ml (mean±standard error) before surgery and 369.1±53.2pg/ml on postoperative day (POD) 1, statistically significant (p<0.05). In Group B, IL-6 was 120.1±59.4pg/ml before surgery and 235.9±57.4pg/ml on POD 1. Group A thus showed a higher value. Mean IL-8 before surgery was 72.7±22.9pg/ml and on POD 1 237.9±13.5pg/ml in Group A, and 56.4±25.0pg/ml before surgery and 41.4±15.6pg/ml on POD 1 in Group B, with Group A showing a significantly higher value (p<0.05). These findings suggest that SAT activates IL-8, perhaps due to hemorrhage or erythrolysis related to collection and the wash system.
