Emdrgency Medical Support Services and Emergency Physicians' Role at Marine Sporting Events.

Background and purpose: Establishment of an emergency system at sports events should be based on the characteristics of each event. However, to date, no study has been conducted to organize the operations of the emergency system at marine sports events. We first established basic procedures for the system and managed the system following these procedures. The purpose of this study is to clarify the necessary conditions for the smooth operation and management of marine sports. Material and method: We examined the results of emergency medical support services provided at seventeen (17) marine sporting events (4 boardsailing, 7 yachting, 6 jet skiing) beginning in 1991. Based on (1) the actual preparation procedure, (2) type of emergency room, and (3) medical record, we examined the following: (A) number of patients and their characteristics, (B) characteristics of patients by sports category, number of patients per day, number of patients transferred to medical institutions, and (C) overview of patients transferred to medical institutions. Results: (1) Emergency teams did follow the basic procedure we established for preparation. (2) An emergency room is an integral part at first stage of the procedure. (3-A) The number of patients who used the service was 557, of which 8.4% were transferred to medical institutions. (3-B) There were notable differences in the characteristics of patient by sports category, but was no difference in the number of patients. Jet skiing showed the most patients who later transferred to medical institutions. (3-C) Most of the patients who transferred to medical institutions had injuries to an extremity with complications. (3-D) No major problems or difficulties in the operation and management of the emergency system were found. Observations: Since the management and operation of the emergency system followed the basic procedure that was established in advance, responding to emergencies could proceed without major difficulties and problems. The medical support teams encountered a variety of conditions and injuries in severity and type. Most important is the development of a coordinated system that can provide first aid, triage and transportation to medical institution. Furthermore, emergency physicians can play more aggressive roles in prehospital care at the site of sports events.
