Relationship between Ventricular Arrhythmia in the Acute Phase of Myocardial Infarction and Short- and Long-Term Prognosis.

Ventricular arrhythmia in the acute phase of acute myocardial infarction (AMI) occurs at high incidence. In this study, we investigated the prognosis of severe ventricular arrhythmia of more than grade 3 in Lown classification during the 24 hours subsequent to the onset of AMI (group B, n=42). The control group had ventricular arrhythmia of less than grade 2 (group A, n=88). With respect to hospital deaths, group B had a significantly higher incidence than group A (57.1% vs 5.7%, p<0.01). The complication of pump failure of more than grade 3 in Killip classification was 75.9% in cases of hospital death. On the other hand, during a long-term follow-up period of 3 years, there was no difference in the incidence of cardiac events between the groups. These results suggest that, the complication of severe ventricular arrhythmia in the acute phase of AMI correlates closely with acute cardiac death, and in particular, the complication of pump failure was related to poor prognosis.
