WATER PRESSURE METHOD TO REDUCE AN INCARCERATED OBTURATOR HERNIA-REPORT OF FIVE CASES-

It has been reported that the reduction of obturator hernia utilizing water pressure (water pressure method) is useful. Since 2002 we have exclusively tried to reduce the hernia by the water pressure method rather than the conventional mannual traction (traction method). Of consecutive ten cases of incarcerated obturator hernia operated on in the hospital during the past six years, the water pressure method was tried in five cases. Bowel resection was necessary for two out of the five cases undergone the water pressure method. Death occurred in one patient and others experienced prolonged hospital stay in the traction method group, whereas all five patients were discharged very much improved within 20 days in the water pressure method group. Based on our experience we suggest that the water pressure method for obturator hernia might improve the therapeutic outcome by avoiding bowel resection, contamination of surgical field, and postoperative wound infection through the protective reduction of the intestine.
