A 72-year-old woman who had undergone a right hemicolectomy for cancer of the ascending colon 3 years before, was detected to have splenic artery aneurysm growing up to 5cm in size. At laparotomy there was no metastatic lesion nor pancreatitis. We detected a splenic artery aneurysm and the proximal and distal arteries. The splenic artery meandered strongly. The splenic aneurysm was opened and the splenic artery was reconstructed by direct ananstomosis. In the treatment of splenic artery aneurysm, aneurysmectomy with splenectomy is thought to be the most common procedure, due to anatomical reasons. However, splenectomy can cause postoperative complications such as pancratic fistula. A arterial reconstruction would be better and simpler than eneurysmectomy with splencetomy.
