A 73-year-old woman was admitted to our hospital because of sudden onset of abdominal pain. Physical examination revealed tenderness, rebound tenderness, and muscular defense in the right lower abdomen. Abdominal CT scan showed a liner high density in the small intestine. The patient had fish meat before onset of the symptoms, we thus made diagnosis of probable local peritonitis due to penetration of fish bone. Because the peritoneal signs were located at right lower abdomen, and ingested fish bone did not perforate the digestive tract, conservative therapy with antibiotics was employed for the patient. On the 3rd hospital day, her abdominal pain was dramatically relieved. Since abdominal CT scan did not show the liner high density any more, it was considered that the ingested fish bone was excreted. Patient was discharged from the hospital on the 13th hospital day. There have been no cases of penetration of small intestine by ingested fish bone successfully treated with conservative therapy like our case in Japan as far as we could review.
