A 72-year-old woman, who had been given steroid for ulcerative colitis, was referred to the hospital with a suspicion of gastrointestinal perforation. Her chest plain X-ray film and abdominal computed tomography showed free air in the abdomen. However, her symptoms and laboratory data did not suggest severe peritonitis. Because a possibility of perforation could not be ruled out, we performed a laparoscopic exploration with gastroduodenoscopy. No causes of pneumoperitoneum could be found out and her pneumoperitoneum was diagnosed as idiopathic.Pneumoperitoneum can occur without peritonitis, however, exclusion of gastrointestinal perforation occasionally seems difficult. In such a case, laparoscopic exploration may be a useful approach.
