A CASE OF GASTRIC METASTASES OF LUNG CANCER PERFORMED A PANCREATODUODENECTOMY FOLLOWING OPERATION FOR THE PRIMARY FOCI

We report a case of gastric metastases of lung cancer performed a pancreatoduodenectomy following operation for the primary foci. A 68-year-old woman admitted to another hospital because of abnormal shadow in the right upper lung field on chest X-rays was diagnosed as having right lung cancer by thorough examinations. She was referred to the hospital and underwent a right upper lobectomy. One year later, tumor markers were elevated and an abdominal CT scan revealed a tumor between the stomach and pancreas. The submucosal tumor with central ulceration was showed by endoscopic examination. With the diagnosis of gastric metastases from lung cancer, she was operated on. The gastric submucosal tumor was about 10 cm in diameter, and involved the pancreas, duodenum, mesocolon and superior mesenteric vein. A pancreatoduodenectomy with a wedge resection of the superior mesenteric vein was carried out. The histopathological examination demonstrated well differentiated papillary adenocarcinoma similar to that of the lung cancer. No postoperative complications occurred and she was discharged from the hospital. She died of peritoneal dissemination nine months after the second operation.
