A CASE OF INTESTINAL OBSTRUCTION CAUSED BY RECURRENT PHYTOBEZOAR FOLLOWING A DISTAL GASTRECTOMY

A 77-year-old man undergoing distal gastrectomy for a duodenal ulcer was admitted to our hospital because of abdominal pain and vomiting. There was a previous history of undergoing enterotomy for an intestinal obstruction caused by a phytobezoar 5 years previously which was associated with the patient's great liking for persimmons. We preoperatively diagnosed the intestinal obstruction as being caused by a recurrent phytobezoar taking further examination and the previous history into consideration. Upon laparotomy, the foreign body was seen to be impacted at the jejunum about 100 cm from the Treitz ligament, and was extracted by enterotomy. The size was 5×4×3cm and it was composed of 98% tannin, so the foreign body was diagnosed as a persimmon phytobezoar. An intestinal obstruction caused by a phytobezoar following gastrectomy is rare and conservative therapy is ineffective. So early diagnosis and early surgery are recommended with precise image inspection, and a regular post operative follow-up is very important.
