Primary amelanotic malignant melanoma of the anorectal region is a rare entity and has a poor prognosis. We experienced a case of amelanotic malignant melanoma of the anorectal region in a 71-year-old man, The patient presented with rectal bleeding. An elastic soft mass was palpated in the anal canal. Proctoscopy showed a reddish tumor. A biopsy specimen was diagnosed as poorly differentiated adenocarcinoma. Abdomino-perineal resection of the rectum was performed. The resected specimen showed the reddish tumor which was of type 1 and 4.5×4.2×1.0 cm indimension. Histologically the tumor was ranked mp, p0, H0, and n1 (+). Although no melanin pigment granules were found microscopically in the tumor cells, they were positive for S-100 protein and HMB-45 monoclonal antibody. This tumor was diagnosed as amelanotic malignant melanoma. He died of liver metastasis 22 months after the operation.
