A Case of Ruptured Splenic Aneurysm Associated with Infectious Endocarditis.

A 40-year-old man complaining of recurrent bouts of fever and body weight loss was referred to the hospital with a diagnosis of infectious endocarditis. He was admitted for operation of mitral valve replacement. During preoperative period, he had a sudden onset of abdominal pain and a loss of consciousness and then developed a shock. Abdominal CT scan and angiography revealed massive intraabdominal bleeding and a ruptured splenic aneurysm. Emergency laparotomy was performed and a resection of the aneurysm with the spleen was made. Post-operative course was uneventful and an elective surgery of mitral valve replacement was successfully performed one month later. Mycotic aneurysm associated with infectious endocarditis has been reported in the systemic arteries. However, solitary aneurysm of the splenic artery is so rare that only three cases have been reported in the literature.
