A Case of Crohn's Disease with Right Iliopsoas Abscess Formation.

A 32-year-old man was admitted to the hospital because of a right lower quadrant abdominal tumor and pain, a fever up to 38.8°C, and a right thigh pain on October 13, 2000. He had repeated symptoms of ileus since 1994, and had been suspected of simple ulcer or Crohn's disease at another hospital. An abdominal computed tomographic scan revealed a low density area with air in the right iliopsoas muscle and he was diagnosed as having iliopsoas abscess. With fasting and administration of antibiotics, his symptoms temporarily improved and a decrease of the tumor was confirmed on CT. However, these symptoms such as high fever and abdominal pain recurred after oral ingestion was started, and so an operation was performed on December 5, 2000. At laparotomy, the terminal ileum was swollen, narrowed, and firmly fixed to the retroperitoneum, but no iliopsoas abscess was found. A partial resection of the ileum was made. The surgical specimen revealed a longitudinal ulcer. Crohn's disease associated with iliopsoas abscess was definitely diagnosed.
