The Surgical Treatment of Recurrent Leiomyosarcoma Originating from Retroperitoneal Space with a Multiple Fistulas between Stomach and Colon. A Case Report.

We report herein a rare case of recurrent leiomyosarcoma of the retroperitoneum which was _??_gically treated. A 45-year-old woman was admitted to the hospital because of fever and general _??_igue. Twenty-nine months prior to the admission, she had undergone a resection of the retroperitoneal _??_nor with distal pancreatosplenectomy, left nephrectomy, and left colectomy in other hospital. diographic examinations on admission revealed: (1) A huge mass, 25cm in size, was located in the _??_roperitoneum, invading the psoas muscle and other adjacent organs; (2) Fistula formation between the _??_nor and colon was confirmed; (3) Deformity of the stomach due to the expansive growth of the mass _??_s also demonstrated; and (4) A single metastatic lesion was demonstrated in the left lateral hepatic _??_e. Extirpation of the tumor including total gastrectomy, partial resection of the small intestine, colon _??_d diaphragm, and lateral hepatic segmentectomy were performed. Pesected specimen also showed _??_tula formation between the tumor and stomach. Postoperative course was uneventfull. She died of _??_patic lesions rapidly extended to the inferior vena cava 8 months after the last surgical treatment _??_owever, she had obtained satisfactory quality of life for at least 6 months postoperatively. To our _??_owledge, there are only four case reports including ours which demonstrated the surgical outcome of _??_current leiomyosarcoma of the retroperitoneum in Japan during these 14 years. This case suggests that _??_ort-term improvement of quality of life can be achieved if removal of the recurrent lesion is possible, _??_hough the validity of the surgical intervention may be controversial in terms of long-term survival.
