We experienced a case of leiomyoma of the esophagus detected at a medical examination for lung cancer. A 29-year-old man demonstrated an abnormal shadow on an indirect chest roentgenogram which was pointed out at a medical examination for lung cancer. Posterior mediastinal tumor was suspected. Computed tomogram (CT) revealed a tumor on the posteior mediastinum. Transbronchial ultrasonic endoscopy visualized a homogeneous hypoechoic extrabronchial tumor. Magnetic resonance imaging (MRI) showed a forward displacement of the trachea. With a fluoroscopic study of the esophagus and a gastric endoscopy, a submucosal tumor 5cm in diameter of the esophagus was diagnosed. The esophageal tumor was removed under 5cm right auscultation triangular thoracotomy and video-assisted thoracic surgery (VATS). Pathologically, it was leiomyoma of the esophagus. This successful removal of the esophageal leiomyoma showing extraluminal outgrowth by a less invasive operation with VATS was reported with some bibliographical comments.
