A CASE OF GIANT EPIPHRENIC ESOPHAGEAL DIVERTICULUM

A 50-year-old woman who had been diagnosed as having esophageal diverticulum due to retarded sensation of foods in 1990 was seen at the hospital because of dysphagia and hematemesis in September, 1996. Esophagoscopy revealed a 6×5cm giant diverticulum in the left wall of intrathoracic lower third of the esophagus on the diaphragm. The patient was thought to be a candidate for operation. Under a left thoracotomy, the diverticulum was excised by using a balloon for dilating the esophagus and Endo-GIA efficiently. Histopathologically, the proper muscular layer was almost replaced by fibrous connective tissue and was seen sporadically like ilands, revealing true diverticulum. No malignant findings were confirmed. Esophagela diver-culum is asymptomatic in most cases and is often incidentally detected by upper gastrointestinal series, and giant diverticulum more than 5×5cm in size is rare. Of these giant diverticular, those with some symptoms or malignant potential only become subjects for treatments.
