A CASE OF EARLY GASTRIC CANCER ASSOCIATED WITH PERNICIOUS ANEMIA

A 67-year-old man was seen at the hospital because of general fatigue. Examination of the blood disclosed macrocytic and hyperchromic anemia, increases in LDH isozyme I and II, an increase in serum gastrins level, and a decrease in vitamin B12. Based on these findings together with positive reactions to anti-gastric wall antibody and anti-intrinsic factor antibody, the patient was diagnosed as having pernicious anemia. Fluoroscopic and endoscopic examinations of the stomach revealed a group of irregular shaped small elevations covering from the posterior wall of the middle portion to angular notch, and it was diagnosed as early gastric cancer of IIa grouped type. Operative procedures included distal gastrectomy and D 1+No 7 lymph nodes dissection. It is believed that gastric cancer with pernicious anemia occurs at a relatively low frequency in Japan compared with those in Western countries. This paper presents a recent case of early gastric cancer associated with perinicious anemia, together with a review of 61 cases seen in the Japanese literature. Gastric cancers associated with perinicious anemia are characterized by elevated lesion, a dominant occurrence of well differentiated adenocarcinoma, no particular common site, and a tendency of multiple occurrence.
