We experienced a rare case of splenic cyst with a high serum CA19-9 level. A 38-year-old man was admitted to the Fuji hospital, complaining of abdominal fullness and constipation. Preoperative blood examination showed a high level of serum CA19-9 (1666U/ml). Ultrasonography and CT of the abdomen showed a large cystic tumor at the splenic hilus. Splenectomy and distal pancreatectomy were performed, as extensive adhesions were present. Intracystic CA19-9 was very high (above 24×104). On immunohistochemical staining with anti-CA19-9, the epithelium of the cyst showed a positive reaction. After the operation, the elevated serum CA19-9 decreased to normal range.
