Hepatic right trisegmentectomy was expected for a hepatocellular carcinoma 10cm in diameter located at the anterior segment in a 69-year-old man with chronic hepatitis C. Preparatively, percutaneous transhepatic portal embolization was performed on the posterior portal branch alone, because the anterior branch had been obstructed with tumor invasion. The lateral segment, future remnant liver, increased in volume by 42.8% to be 552cm3 during 7 days. The patient underwent a hepatectomy 8 days following portal embolization and was discharged from the hospital on the postoperative day 15, uneventfully. Such a rapid hypertrophy of the future remnant liver by preoperative portal embolization is rare that permits to shorten the period for treatment.
