We experienced a case of advanced breast cancer which was successfully treated with palliative surgery using a musculocutaneous flap and chemotherapy based on histoculture drug response assay (HDRA). A 64-year-old woman was seen at the department because of dyspnea and ulcer of her left breast. Under a diagnosis of advanced breast carcinoma with lung and liver metastases, she was treated with tamoxifen and CEF chemoendocrine therapy (cyclophosphamide, epirubicin and 5-FU). As the tumor regressed only a little by the chemoendocrine therapy, we carried out incisional biopsy to perform HDRA. Based on the results of HDRA, she was administered combination chemotherapy added mitomycin, which regressed the tumor partially (PR). Therefore, the local tumor was palliaively resected using musculo-cutaneous flap. The patient also received postoperative adjuvant chemotherapy containing mitomycin and has been alive without progression of the disease for more than one year.
