This paper describes a recent experience with successful cadaveric renal transplantation in a 39-year-old man using a cadaveric kidney donated from a 5-year-old boy. The recipient had a good graft survival and was able to be freed from hemodialysis. The recipient had a 2-year and 4-month history of receiving hemodialysis. The compatibility between them was from HLA4 match, O(+) to AB(+). The donated kidney was 80g in weight and was transplant-ed at worm ischemic time of 22 min and total ischemic time of 17 hrs and 25 min. On and after the second day following the transplantation urination was observed. No rejection nor any complications occurred. As of 1.5 months after the operation the s-Cr value has been kept at around 1.4mg/dl. With an improvement in the renal function the graft showed an increasing tendency from 7.6 to 10.2cm in longer diameter on echography. It is rare in Japan that a kidney from a cadaveric infant dormer is transplanted to an adult recipient. Some foreign reports present that there were differences in the graft survival. We also describe a review of the literature.
