A CASE OF LEIOMYOBLASTOMA OF THE ILEUM WITH MELENA AND HIGH FEVER

A case of leiomyoblastoma of the ileum is reported. A 65-year-old male was admitted to Imabari Hospital because of melena and high fever in October 1992. A diagnosis of malignant lymphoma of the ileum was made based on the results of a physical examination, barium study of the upper gastrointestinal tract, abdominal computed tomography, abdomi-nal angiography and Ga scintigraphy. The tumor of the ileum was surgically resected in December 1992. It was localized in the ileum 80cm proximal to Bauhin's valve, and was tightly adherent to the peritoneum and the bladder wall, but no metastatic lesions were found. The cut surface of the tumor was pinkish white. An abscess cavity had formed in the center of the tumor, and it had perforated into the lumen. Histologically the tumor cells consisted of spindle-shaped cells and cells with a characteristic epithelioid appearance. Mitotic figures were rare. It was possible to completely remove the tumor. The patient was discharged without chemotherapy, and had no signs of recurrence 24 months after surgery. Leiomyoblas-toma of the intestine is rare as a preoperative diagnosis. However, we believe that it is possible to diagnose leiomyoblastoma of the intestine preoperatively in patients complaining of melena and high fever, as in this case.
