A CASE OF ILEAL CARCINOID TUMOR PRODUCING INTUSSUSCEPTION

A case of ileal carcinoid tumor presenting with intussusception was reported. A 26-year-old female was seen at the hospital because of 3-month lasting right lower abdominal pain. On physical examination, a movable mass was noted at the right lower abdomen. Characteristic findings of intussusception were recognized on barium study, ultrasonography and CT scan. Colonoscopic examination revealed an ileal tumor which was intruding into the ascending colon and moving back and forth. In operation, ileocaecal resection with regional lymphadenectomy was carried out after obtaining diagnosis of carcinoid tumor. No lymphatic or liver metastasis was observed. Carcinoid tumor of the small intestine is rare in Japan. The most common clinical presentation is long-standing periodic abdominal pain. Intussusception is distinctly uncommon. Multiple synchronous tumor are occasionally observed. The surgeon should palpate the small bowel for synchronous tumor to be resected. Because the incidence of lymph node metstasis is quite high even in the small-sized tumor, regional lymphadenectomy is recommended.
