CLINICOPATHOLOGICAL STUDIES ON MUCIN-PRODUCING CYSTIC TUMORS OF THE PANCREAS

A retrospective study has been conducted on 10 cases of mucin-producing cystic tumor of the pancreas (MCT) experienced at the hospital from January 1984 to March 1992. In this study clinical findings, some problems of operative procedures and those presented from postoperative follow-up were discussed. In a pathological study, these were 4 cases of mucinous cystadenoma, 4 cases of mucinous cystadenocarcinoma, and 2 case of so called mucin-producing pancreas cancer. The average age of the patients was 57.2 years, and the male to female ratio was 5:5. The tumor was located in the pancreas head for five cases, in the body for three, and in the tail for two. Five cases underwent pancreatoduodenectomy (PD) and, another five, distal pancreatectomy. Radiological features and tumor maker could not offer any clues to differenciate benign from malignant. Over 3cm in diameter of the cyst, and septum formation or solid component existent in the cyst should be received surgical treatment. Decision for the resected line of the pancreas needs intraoperative careful diagnosis by frozen section, because diffuse infiltration of so called mucin-producing pancreas cancer is common. Pyrolus preserving PD should be good indication for the MCT which located in the pancreas head area, but re-construction method should be chosen for consideration of local recurrence. MCT recurrence can be resected again if it was found in an early stage, so careful follow up would be mandatory in these low malignant diseases.
