A case of pseudomyxoma peritonei caused by a primary carcinoma of the appendix presenting with an umbilical tumor is described. A 68-year-old male was admitted to the department of an enlarging umbilical tumor. The patient's abdomen was remarkably distended, and the umbilicus, 10cm in diameter, protruded outside, with partial redness of the skin. Abdominal simple x-ray examination revealed a tumor shadow beneath the skin of the protruded umbilicus. bdominal ultrasonography and CT scan showed multiple septations in the umbrical tumor, fluid collections and a cystic tumor in the right lower quadrant. Barium enema study demonstrated the extrinsic compression of the cecum. These findings suggested pseudomyxoma peritonei from a primary carcinoma of the appendix which was presenting an umbilical tumor. In the operation, the umbilical tumor, swollen appendix and mucinous fluid were removed. Microscopically, mucinous adenocarcinoma of the appendix was identified, and only mucinous collections were found in the umbilical tumor. After the operation, chemotherapy 5-FU was performed. The patient is doing well for six months following the surgery.
