A STUDY OF SURGICAL RESULTS OF MALIGNANT GASTRIC LYMPHOMA AND FACTORS AFFECTING ITS PROGNOSIS

We studied clinicopathologically 22 cases of primary gastric lymphoma and evaluated the factors affecting the survival rates after operation. Seventeen patients underwent curative operation and 3 patients underwent noncurative operation; the resectability was 91%. Macroscopically there were 2 cases of type 2c, 2 of Borrmann type 2, 16 of Borrmann type 3 and 2 of Borrmann type 4. Mean size of the tumors was 10.4±4.8 cm. Lymph node involvement was observed in 4 of 8 cases with ps (-) and 6 of 13 cases with ps (+). Four cases were categorized into stage 1, 3 cases stage 2, 10 cases stage 3, and 5 cases stage 4. Histologically the majority was classified as diffuse type, including 12 cases of diffuse large type and 6 of diffuse medium type. The 5-year survival rate of curative operation was 43.2%, and 0% for noncurative operation with a significant difference. Adjuvant chemotherapy using CHOP changed the survival rates, however, radiation therapy did not. Clinicopathologically the prognosis of cases in stage 1, ps (-) or growths less than 14 cm was relatively good, but lymph node involvement or histological type did not affect the survivals.
