Adenocarcinomas which form cysts in the liver are looked upon as cystic adenocarcinomas, and 40 cases are seen in the Japanese literature up to now. There are not a few reports in which a cancerated hepatic cyst is misunderstandingly categorized into a cystadenocarcinoma. Cystadenocarcinomas can be classified into 2 types etiologically, namely, (1) cancerated cystadenomas and (2) cystadenocarcinomas from its occurrence on; and it should be distinguished from the cancerated hepatic cyst in terms of clinical course and histopathologic features. Here we describe a case of the disease which might occur as a cystadenocarcinoma. A 52-year-old male complained of right hypochondralgia and fever. Abdominal ultrasonography, abdominal CT, and ERCP offered suspicions of choledocholisis and left hepatic stones. Left lateral segmental excision and excision of common bile duct with T-tube drainage were carried out. During operation, jelly-like muciform substance was collected from the common bile duct, however, no stones could be found. After surgery a diagnosis of cystadenocarcinoma was made based on histopathological findings.
