Acute suppurative thyroiditis is an uncommon disease and its possible route of infection had been in debate until recently. However, Miyauchi and others proved that acute suppurative thyroiditis results from infection through the piriform sinus fistula.This paper describes three cases of acute suppurative thyroiditis we have experienced, and in all of them a sinus fistula was demostrated by a barium meal rentogenogram performed prior to surgery. The sinus fistula was observed in the left side in 2 cases and in the both sides in one case. Two fistulas were comfirmed during surgery, and in all cases fistulas could be removed completely. No recurrent cases have been seen so far.
