STUDIES ON THE CLINICAL COURSE OF ACUTEABDOMEN WITH ENDOTOXEMIA

Ninety-eight patients who had undergone surgical procedures during the period from April 1975 to December 1984 were diagnosed as acute abdomen with endotoxemia. Of these patients, 74 were divided into three groups by the clinical course. The 13 in group A showed circulatory shock level at the initial visit and survived as a result of surgical procedure. The 28 in gooup B showed shock at the initial visit and died during the early postoperative period. The 33 in group C showed no clinical shock symptoms and survived postoperatively. A comparative clinical study of these 74 patients to examine the changes of parameters for the function of im-portant organs in the course of time among the three groups, and the results were evaluated to reach the following conclusions. The concentration of blood endotoxin level in both groups A and B was high at the initial visit as compared with at the onset of shock. Post operatively, the clinical status was improved with the disappearance of endotoxemia in groups A and B. However, endotoxemia continued in group C. Respiratory impairment was the most common complication in these patients and renal failure, hepatic dysfunction and gastro-intestinal bleeding were also frequently seen in groups A and B. Estimating from the clinical course, improvement in the hemodynamics within six hours postoperatively brings about good results in the subsequent clinical course, constituting a major factor in the improvement in prognosis. The particular importance of correcting the hemodynamic state before the operation was suggested.
