Acute Myocardial Infarction Secondary to Blunt Chest Trauma

A 47-year-old man had acute myocardial infartion after chest trauma caused by an automobile accident. Physical examination on arrival revealed blood pressure of 102/74mmHg and regular pulse rate of 73 beats per minute. Abrasions on the anterior chest wall were noted. A chest X ray revealed bilateral pulmonary congestion. ST elevation in leads I, aVL and V1-3 was seen and ST depression in leads II, III, aVf and V4-6 were seen in an electrocardiogram. Coronary arteriography showed complete occulusion of the left main coronary. An intra-aortic balloon was therefore inserted and counterpulsation was initiated. Emergency surgery was performed with the use of intra-aortic balloon pumping. During the operation, rupture due to blowout of a tear in the anterior wall of the left ventricle occurred. The tear was repaired with a patch and then, left internal mammary artery bypass grafting of the arteriotomy site in the left descending artery was performed. The patient died due to sepsis and multiorganic failure 34 days after the operation.
