Analysis of the Prehospital Care Provided by Doctor Car in Our Institute for Patients with Head Trauma

Purpose: To clarify whether our doctor car system is useful or not for the effective treatment of severe head injury patients. Methods: We retrospectively investigated head injury patients who were transferred by a doctor car between July 1993 when this system had started and October 2003. Results: Among 814 patients who were transferred by the doctor car, sixty-eight had head injuries (subject). The average age of the subjects was fifty years old, and forty-eight subjects were male. There were 14 cardiopulmonary arrests at the scene and 9 of 14 could not undergo cardiopulmonary resuscitation due to the severity of injuries. Excluding the cardiopulmonary arrest cases at the scene, twenty-three subjects were transferred to local hospitals and obtained good results. Of the nine subjects who received treatments at the scene, mainly consisting of tracheal intubation, four subjects died in spite of aggressive prehospital care. Conclusion: Because the chance to perform aggressive prehospital care for severe head injured patients is small, our system might need improvement.
