Epidemiological Investigation of Cardiovascular Disease in Emergency Department in Tokyo

AIM: The effectiveness of cardiovascular-related emergency medical systems may not be well established or organized yet, and is open to further discussion in Japan. To establish and deliver high quality medical care for the general population, epidemiological information and emergency patient data based on the symptoms is indispensable and is warranted for the planning and re-structuring of the emergency medical system. The aim of the present study was to determine and study the incidences of symptoms associated with cardiovascular disease and also to define the prevalence of cardiovascular disease in emergency patients who were acutely ill. METHODS: Between 1989 and 2001, 21, 213 patients were brought to the emergency department of the Keio University Hospital in Tokyo by ambulance because of variable acute illnesses. Among them, the incidences of patients presenting with symptoms associated with cardiovascular diseases (chest and/or back pain, dyspnea, palpitations or transient loss of consciousness) were studied and determined. Furthermore, the prevalence of cardiovascular diseases among the patients with those cardiovascular symptoms was also determined and analyzed. RESULTS: Of the 21, 213 patients, 5, 661 patients (26.7%) had symptoms related with cardiovascular diseases. The percentages of patients with chest and/or back pain, dyspnea, palpitations, and transient loss of consciousness in the patients with acute illness were: 5.9%, 5.9%, 2.1%, and 12.8%, respectively. The prevalence of organic cardiovascular disease among the patients complaining of chest and/or back pain, dyspnea, and palpitations was 48.1%, 14.1%, and 31.6%, respectively. Of the patients complaining of transient loss of consciousness, 78.8% had syncope, and 4.4% were diagnosed as having cardiac syncope. CONCLUSION: More than 25% of the patients transported to the emergency department by ambulance for acute illness had symptoms related with cardiovascular disease. However, the prevalence of cardiovascular disease among these patients was less than 50%.
