Urgent laparoscopic removal of a fresh intraperitoneal escaped needle was successfully performed in a 25-year-old man. The patient had beed treated as schizophrenia for 10 years. He had enjoyed passing through his abdomen by several needles and 10 days later he was referred to our hospital because of abdominal pain. Abdominal plain X-ray, ultrasound and CT revealed an intraperitoneal escaped needle with no intraperitoneal bleeding. Urgent laparoscopic retrieval was performed with the 2-port method (umblilical port for laparoscopy and upper abdominal port for forceps and scissers). We found the end of a needle which had been capsulated into the omentum. The needle was easily removed by laparoscopic forceps and adhesion between the omentum and the anterior abdominal wall was released. It took only 41 minutes to finish the operation. The patient was discharged from the hospital on fifth day after the operation. Laparoscopic retrieval may be the treatment of choice for an intraperitoneal escaped needle with no intraperitoneal bleeding.
