A very rare case of malignant lymphoma, so-called "Lennert's lymphoma", arising in the spleen is reported in this paper. A 63-year-old male having a bladder cancer was admitted to the hospital for the operation. Abdominal CT scan and abdominal ultrasonography performed preoperatively revealed an unexpected solitary lesion in the spleen. We could not determine whether the lesion was a tumor originated in the spleen or it was a metastatic tumor from the bladder cancer. The patient underwent splenectomy and total cystouretherectomy at the same time for the diagnosis as well as the treatment. The resected spleen showed no enlargement and the tumor was confined to the spleen. Pathohistologically the tumor was non-Hodgkin's lymphoma, peripheral T cell lymphoma with a multifocal epithelioid granulomatous lesion, so-called "Lennert's lympoma". This is a very rare type of malignant lymphoma, and it is hard to be classified according to the LSG classification or WF. Especially the case arising primarily in the spleen has never been reported in Japan. Although the patient has no recurrent sign as of five months after the surgery, should be given some adjuvant chemotherapy.
