Dietary Control of Chronic Renal Failure with Special Reference to Amino Acid Metabolism

Low protein diet has been recommended since many years ago for patients with chronic renal failure. However, a considerable number of patients developed malnutrition during their clinical course. Since chronic dialysis and renal transplantation remarkably contributed to prolongation of the patients' life, the problem of food is considered to be a critical factor in order to give a comfortable life to the patients and to avoid malnutrition syndrome to appear in them.<BR>It was pointed out that a significant amount of amino acid is also dialyzed across the cellophane membrane. The authors measured individual amino acid contents in plasma of the patients and the dialysate by means of an auto amino analyzer. By a Kolff type dialyzer, 57.3 mg/hr of total amino acid is lost to dialysate, whereas 20.0 mg/hr is filtered by a Kiil type dialyzer.<BR>Among the plasma amino acids, lysine, aspartic acid, threonine and valine are lost during hemodialysis. Therefore, either protein of the high biological value or essential amino acid supply should be taken into consideration for the patients undergoing chronic dialysis<BR>In addition, various regimen were investigated for the patients with chronic renal failure. At the present time, it is concluded that no clinical improvement was observed if creatinine clearance was below 5 ml/min. However, if creatinine clearance is higher than 5 ml/min, a 30 to 40 g. protein diet with 2,000 calories per day appears to be recommended for the patients undergoing chronic dialysis. Whether protein of the high biological value or essential amino acid should be provided remains unclear.
